(2008-08-25)

Both Lafayette hospitals made the list. St. Elizabeth Medical Center reported three errors out of its nearly 9,000 procedures, and Home Hospital also had three problems in about 18,000 procedures.

Home Hospitals issues were more severe, which includes a death or a serious disability associated with a low risk pregnancy or delivery.

Severe bed sores, foreign objects left in bodies post-op and surgeries performed on the wrong body part topped the list statewide, according to the Department of Health.

St. Vincent Hospitals in Williamsport and Frankfort, St. Clare Medical Center and White County Memorial Hospital were error-free according to the Department of Health.
