Illnesses can strike at any time and it may be too late to find out what your loved ones want, or communicate your own wishes. Ensuring Your Care Decisions are Followed introduces the Five Wishes advance directive and the importance of creating one. Attendees will also learn how to begin this important conversation with loved ones.
